Before your first visit, please print and fill out the following patient forms and bring them with you:
- Consent for Treatment
- Adult Health Information Form or Child Health Information Form
- Insurance/Patient Info Form
- Notice of Privacy Practices – Acknowledgment
- Medical Privacy Form – this is for your records; you do not need to bring it in
Your first visit will take anywhere from 1.5 to 2 hours to give Dr. Shinsato an opportunity to learn your medical history, understand your current concern, and perform a physical exam. We may order laboratory and diagnostic tests, if necessary.
Please plan on arriving 5-10 minutes before your visit so we can start on time. Directions
Primavita Family Medicine accepts cash, personal checks, health savings account cards and major credit cards. Payment plans
If you have health insurance, please call my billing company at 877-776-2311 and we can determine your benefits. I am currently an in-network provider with Premera, Lifewise, Regence, Aetna, Cigna, and First Choice Health. I am on the waiting list for Group Health (it is currently closed to naturopathic doctors and acupuncturists).
We will be happy to fill out your health insurance form so you can submit it to your insurance company for reimbursement for out-of-network benefits. Please note that payment is required at the time of your visit. Worksheet to determine your out of network benefits
For patients without health insurance or with a high deductible insurance, we offer a convenient 50-50 Health Plan, which provides you with cost-effective healthcare starting at $50 a month.
